چکیده

Abstract Lactate clearance is affected by hepatic function. However, it unclear whether the association between dysfunction and lactate can act as a prognostic marker of clinical outcomes in patients with septic shock. We aimed to evaluate mortality two cohorts shock patient who had based on their total serum bilirubin levels (TBIL). at 24 h after onset was analyzed using cohorts, sub-categorized into groups TBIL: < 2 mg/dL ? mg/dL. In derivation cohort, lower non-survivors than survivors TBIL mg/dL, while there no significant difference Multivariate logistic regression analysis revealed that increased significantly associated decreased 28-day group (10% clearance, adjusted odds ratio [OR]: 0.88, 95% confidence interval (CI): 0.80–0.97, P = 0.0075), Creatinine level (adjusted OR: CI: 0.81–0.95, 0.00069) APACHE II score 35 0.93, 0.87–0.98, 0.013). validation observed Increased 0.89, 0.83–0.96, 0.0038) just about 0.86, 0.74–1.00, 0.051). conclusion, (TBIL mg/dL) or high severity (APACHE 35) mortality.

Is lactate clearance impaired in septic shock?

In a recent article in Critical Care, Tapia and colleagues evaluated the relationship between exogenous lactate clearance and liver perfusion in a short-term model of endotoxemic shock [1]. The authors concluded that the very low portal-hepatic vein lactate gradient was indicative of the inability of the liver to metabolize the increased lactate load. Although we believe that the authors addres...

Early lactate clearance is associated with improved outcome in severe sepsis and septic shock.

OBJECTIVE Serial lactate concentrations can be used to examine disease severity in the intensive care unit. This study examines the clinical utility of the lactate clearance before intensive care unit admission (during the most proximal period of disease presentation) as an indicator of outcome in severe sepsis and septic shock. We hypothesize that a high lactate clearance in 6 hrs is associate...

Prognostic significance of blood lactate and lactate clearance in trauma patients.

BACKGROUND Lactate has been shown to be a prognostic biomarker in trauma. Although lactate clearance has already been proposed as an intermediate endpoint in randomized trials, its precise role in trauma patients remains to be determined. METHODS Blood lactate levels and lactate clearance (LC) were calculated at admission and 2 and 4 h later in trauma patients. The association of initial bloo...

Homocysteine levels in septic shock

Methods Prospective cohort study of patients admitted to the Intensive Care Unit with criteria of septic shock (SCCC 2001), mechanically ventilated and requiring vasoactive drugs. Demographic variables, haemodynamics, respiratory, renal function, CRP and lactate were collected. HCY levels, coagulation factors II, VII, VIII and XII were determined at admission (T1), at 48 (T2) and 72 hours (T3),...
